Out-of-Network Provider: 

As a licensed independent clinical social worker, my professional services may be eligible for reimbursement as an out-of-network provider for all other insurance plans.  This means that my services may be covered in part by your health insurance or employee benefit plan. You would pay me in full at the time the service, and I would provide you with an insurance statement that you can submit to your insurance company for out-of-network reimbursement. 

Working out-of-network allows for an increased level of confidentiality and scope of treatment, allowing for more creative freedom to meet your needs over that of insurance companies.  

If you plan to use insurance, please contact your insurance provider directly by calling the customer service number on the back of your insurance card and ask the following questions of them: 

• Do you have mental health insurance?
• What is my deductible for an out-of-network provider and has it been met?
• How many sessions per year does my health insurance cover?
• What is the coverage amount per therapy session? 
• Is approval required from my primary care physician?

Please note that insurance companies require you to authorize me to provide them with a clinical diagnosis.
